4x4 Debugging Framework

A four-step escalation sequence for resolving a stuck AI coding agent: first let the tool attempt its own automatic fix, then manually add logging to surface awareness of the problem, then hand the issue to an external diagnostic AI tool for a second opinion, and finally revert to an earlier working version and retry with a clearer prompt; each step is attempted only once before moving to the next.
